Honestly, I don't give a damn what doctors/nurses/PAs/GPs/etc do on their free time but I would expect them to adhere to basic courtesy and not smoke/drink alcohol when their working. Not everyone is going to care one way or the other but, like I mentioned in my previous post, others will be allergic to said smoke or even to the smell of alcohol. Or a nurse will get a patient who will throw a fit that their nurse smells like cigarette smoke and go on a complaining rampage about it.
Not everyone is going to want a doctor, nurse, or whoever that smells like they had four cigarette's take their blood or be in the room with them, either -- some may not care one way or the other, but there may be one or two or a dozen patients that wants a new doctor or nurse because they don't want to smell the cigarette smoke on the person who's supposed to be taking care of them.
I work with co-workers who smoke, one former co-worker who had three cigarette's every twenty minutes -- most of my current co-workers have the common courtesy to smoke on their breaks and not smell like they just smoked a full pack. The one former co-worker I mentioned above smelled awful just from the nicotine on her clothing, I'm surprised no customers complained about it.
( side note: I work with someone who is allergic to alcohol, she can't go to any restaurants that sell alcohol because even smelling it causes her to break out into a sweat and she gets red in the face. I don't know what cigarette smoke does to her or if she's allergic to it, but I assume that if she were she wouldn't be able to work in retail for long )
