I hardly saw a dentist as a kid, myself (not that it would have helped >_< ), and my teeth were still white. This isn't about not seeing a dentist - this is about parents not cleaning their kids' teeth at all.

That's what I'm thinking. My family had to go a while without professional dental care, due to the recession / lack of insurance / difficulty finding jobs. But our teeth never got that horrible.

Toothbrush, toothpaste, dental floss. Not expensive at all, no insurance needed.
